Case Management Program (HIV/AIDS)
The HIV Case Management / Continuum of Care Program serves individuals living
with HIV/AIDS [PLWHA] and who reside in Arenac, Clare, Gladwin, Isabella, Osceola, and
Roscommon counties, the public health jurisdiction served by CMDHD. Persons
identified as HIV positive, and residing in adjacent counties, also have an
opportunity to be served by CMDHD case management personnel. Services provided
are:
- Ambulatory/Outpatient HIV Medical Care
- Case Management & Client Advocacy Services
- Direct Emergency Financial/Client Assistance
- Health Education & Risk Reduction Services
Healthy Start - Gladwin
Provides assessment, home visitation and support services to Gladwin County
families who have an infant under 12 months of age. The Healthy Start Program
offers education to parents on a variety of topics (i.e. nutrition, immunizations,
infant development). This program focuses on each family's needs. Eligibility
requirements do apply.
Partner Services
Partner Services provides a combination of efforts: bringing partners and/or
contacts to counseling in a confidential manner, testing partners and/or contacts,
informing individuals about treatment and who have contracted Syphilis and/or are
HIV positive. In the case of an individual having tested positive for HIV, additional
client-centered services are offered; HIV Case Management Services are explained and
referral information, naming those agencies and organizations offering such services,
is provided to the client. Central Michigan District Health Department (CMDHD)
personnel provide Partner Services in 62 Michigan counties.
